Inclusive Medical Services. Inc. (AIMS) is a medical practice group of physicians licensed in the state of California. It is a unique group of physicians and practice extenders providing management services for primary care along with wounds and infection in the nursing home setting. AIMS is comprised of board-certified physicians in Geriatrics, Wound Care, Pain Management, Infectious Disease and Nurse Practitioners. With over 10 years of serving Nursing Homes and meeting Medicare guidelines for physician level of documentation in the facility setting, AIMS has unmatched experience and focus in providing the highest quality of care and outcomes.

OUR GOALS:

  • Improve quality of care for patients at SNFs
  • Reduce re-hospitalization rate
  • Establish a post-acute network with intermediate facilities

OUR SERVICES:

  • On-site presence of Nurse Practitioner under MD supervision
  • Help reduce readmission to the hospital and improve medication compliance with Home Visits services
  • Regular rehab meetings for skilled nursing patients
  • 24 hour / 7 days per week call coverage including, but not limited to, emergency coverage
  • COPD / CHF and pain management programs
  • Tools / protocols (i.e. INTERACT, abnormal lab values, etc.) for managing patient’s care
  • Active participation in monthly / quarterly QA meetings
  • Cost Efficiency working with facility’s consulting pharmacist to positively impact cost of medications
  • In-service to nursing staff in collaboration with facility administration
  • MD visits for all skilled nursing patients during their in-patient stay at the SNF while monitoring them for acute / change of condition visits
  • Focus on SBAR to reduce unnecessary ER transfers
  • Early detection of acute changes in patients by increased presence of NP / MD. thereby, significantly reducing hospital re-admissions
  • Working closely with DONs / Administrators for Quality Indicators to help improve Medicare star ratings
  • Implementing protocols to improve quality of care e.g. protocols to reduce bedsores for high-risk patients
  • MD involvement in training RNs / LVNs, while developing good communication, to work as a team
  • Non-traditional way of practicing medicine in SNFs by fellowship trained Geriatrician following current and updated guidelines
  • Help the facility develop medical information and communication system with staff, patients, and families
  • Help establish appropriate relationships with other healthcare organizations
  • Help with financial issues by working with the facility’s consulting pharmacist, making a positive impact on the facility’s cost of medications and assisting the facility in meeting its financial goals by accurately determining
